Create Custom CSV Layout Screen

 Purpose

The purpose of the Create Custom CSV Layout screen is to allow a provider to specify its own flat file format for loading patient, immunization, comment, and event files into the ALERT IIS.  Training and experience with Data Exchange is required to use this feature.

 View Custom CSV Layout

To view a custom csv layout, follow these steps:

  1. Select Custom CSV File Layouts option under the Data Exchange menu Group.

  2. Select the Custom CSV File Template Name from the Custom CSV File Template Name Pick List.

  3. Click the View button.

 Create New Custom CSV Layout

To create a new custom csv layout, follow these steps:

  1. Select Custom CSV File Layouts option under the Data Exchange menu Group.

  2. Click the Create new layout button.

  3. Enter the Template Name.

  4. Select whether a Header row will be included with the CSV File.

  5. Select the fields and order of how the fields will be specified for the CSV files.

  6. Select whether the template will be used Globally or just for the provider organization by using the Global Template pick list.

  7. Click the Save button.

 Edit Custom CSV Layout

To edit a custom layout, follow these steps:

  1. Select Custom CSV File Layouts option under the Data Exchange menu Group.

  2. Select the Custom CSV File Template Name from the Custom Flat File Template Name Pick List that needs to be edited.

  3. Click the Edit button.

  4. Change the Template Name.

  5. Change where a Header row will be included with the CSV File.

  6. Change the fields and order of how the fields will be specified for the CSV files.

  7. Change whether the template will be used Globally or just for the provider organization by using the Global Template pick list.

  8. Click the Update button.

Template Name of the custom template.

Record Identifier

Record Identifier, used to link a Patient to Immunization, Comment or Event records in the Patient file.

First Name

First Name of the Patient in the file.

Last Name

Last Name of the Patient in the file.

Birth Date

Birth Date of the Patient in the file.

Record Identifier

Record Identifier, used to link an Immunization to a Patient.

NDC Code

NDC Code of the Immunization File, either NDC Code, Trade Name, CPT Code, CVX Code or Vaccine Group need to be entered.

Trade Name

Trade Name of the Immunization File, either NDC Code, Trade Name, CPT Code, CVX Code or Vaccine Group need to be entered.

CPT Code

CPT Code of the Immunization File, either NDC Code, Trade Name, CPT Code, CVX Code or Vaccine Group need to be entered.

CVX Code

CVX Code of the Immunization File, either NDC Code, Trade Name, CPT Code, CVX Code or Vaccine Group need to be entered.

Vaccine Group

Vaccine Group of the Immunization File, either NDC Code, Trade Name, CPT Code, CVX Code or Vaccine Group need to be entered.

Vaccination Date

Vaccination Date of the Immunization File.

Record Identifier

Record Identifier, used to link a Comment to a Patient.

Comment Code

Comment code of the Comment File.

Record Identifier

Record Identifier, used to link an Event to a Patient.

Event Code of the Event File.

 Functionality

The following table describes the buttons available on the Create Custom CSV Layout screen.

Command Button

Description

When clicking on the View button, the system displays the detailed field information and order for all the fields in the custom CSV layout chosen from the pick list.

When clicking the Edit button, the system displays the Edit CSV Layout page.

When clicking the Create new layout button, the system displays the Create New CSV Layout page.

When clicking the Save button, the system validates all required fields are entered correctly and saves the New CSV Layout that was developed.

When clicking the Update button, the system validates all required fields are entered correctly and saves the edited CSV layout that was developed.

When clicking the Cancel button, the system returns to the previous screen.
