Patient AKA Section |

Purpose |
The Patient AKA section is used to add patient AKA Names with two sections described below.

Patient AKA Listing |
This section displays a table described below with all of the Patient AKA Names for the patient.

Remove AKA |
This column displays only for State Users and displays a check box in order to remove AKA Names for patients. |
|
Last Name |
This column displays in each row the last name for the AKA Name in that specific row. |
|
First Name |
This column displays in each row the first name for the AKA Name in that specific row. |
|
Middle Name |
This column displays in each row the middle name for the AKA Name in that specific row. |
|
Add New AKA |
This section allows users the ability to add AKA Names to a patient using the following fields described below.

Last Name |
This is a required field and is the Last Name of the AKA Name for the patient. |

First Name |
This is a required field and is the First Name of the AKA Name for the patient. |

Middle Name |
This is the Middle Name of the AKA Name for the patient. |

Command Button Functionality |

Description |

Clicking the Add AKA button will validate the AKA Name entries in the AKA Name text boxes and then add the AKA Name record to the Patient Record. |

|
Clicking the Remove AKA button will remove selected AKA Names from the patients that have check boxes selected in the Remove AKA column. This button is only available to specific State Users. |
