Purpose |
Upload List Screen is to be used in mass vaccination events and will upload a list of patients or update a list of patients in order to be able to vaccinate multiple patients at the same time.

Information Provided |
A provider can either specify a New List Name by entering a New List Name in the New List text box or can update an already existing list by selecting a list from the Update List drop down box.

Functionality |
Patient List Information is passed to the central system using a single upload file, respectively the screen is described in the table below.

Specify a new name for the list being created by the uploading the list file. |

Update List |
Select a previous list to update by uploading the list file. |

File Name |
Enter the file path for the List File on the users computer. |

Clicking the Browse button will display a windows explorer window in order to be able to find a file on the user's computer to submit for uploading a list. |

Clicking the Upload button will start the upload list process for the information that has been entered into the fields on this page. |
