Patient Address Information Section |
Purpose |
The Patient Address section is used to manage a patient's address, phone number, and e-mail address. The ability to make corrections to an existing address or record a new address in case the patient has moved is allowed in this section. The patient's address history may also be viewed from this section.
Make Address Correction |
To correct a patient's current address, follow these steps:
Click on the Address Information section header.
Make the correction directly to the current address which displays for the patient. Do not change or edit the start date.
Click the Save button. The system saves your correction. You may verify this if you go to View the Address History
Record Patient Move |
If the patient has moved, you may want to record a new address. To do so, follow these steps:
Click on the Address Information section header.
Enter a start date for when the new address the patient moved to became effective.
Enter the new address information, overwriting the values for the current address.
Click the Save button. The system saves your correction. You may verify this if you go to View the Address History
View Address History |
To view the patient's address history, follow these steps:
Click on the Address Information section header.
Click the View Patient Address History hyperlink.
The system displays a listing of all the addresses the patient has ever lived at and the start date for when each address became effective.
Click the Save button. The system saves patients new address. You may verify this if you go to View the Address History

No Viable Address |
Check this box if the current address showing for this patient is no longer valid and no new address is known for the patient. Checking this box prevents the system from sending reminder recall letters to the patient. |
Start Date |
The start date indicates when the patient started to live at the address. |
Street Address |
The first line of the street address for the patient. |
Other Address |
The second line of the street address information for the patient, which may include apartment, room, or suite numbers. |
P.O. Box |
The patient's post office box number. |
Zip |
The patient's 5-digit postal zip code. One significant feature of this field is that you may enter any Oregon zip code, click Save and the system will automatically populate the city, state, and county fields for you. Please note that you must leave the city and county fields blank in order for this feature to work. |
Zip+4 |
The 4-digit zip code extension for the patient. |
City |
The patient's city. |
State |
The patient's state. |
County |
The patient's county of residence. |
Phone Number |
The patient's phone number. |
Extension |
The patient's phone number extension. |

The system only accepts valid dates. The system allows only dates on or after the patient's birth date and on or before today's date. |

The system allows alpha or numeric characters, dashes, periods, apostrophes, forward slashes '/', and pound symbols '#'. |

The system allows alpha or numeric characters, dashes, periods, apostrophes, forward slashes '/', and pound symbols '#'. |
P.O. Box |
The system allows alpha or numeric characters, dashes, periods, forward slashes '/', and pound symbols '#'. |
City |
The system only accepts alpha characters, dashes, and apostrophes. |
Zip |
The system only allows numeric characters. |
+4 |
The system only allows numeric characters. |
