• Order Confirmation Section- This section displays Organization information that is specific to the Provider Organization Ordering Vaccines as described in the table below.

• The order confirmation page is broken down into three sections:

• • Pediatric State-Supplied Vaccine- In this section the user will see the specific Order quantities for their pediatric state-supplied vaccines.  

  • Adults State-Supplied Vaccine -In this section, the user will see the specific Order quantities for their adult state-supplied vaccines.

  • Ancillary - In this section, the user will see the specific Order quantities for any Ancillary vaccines.  

  • Below each individual section, the user will see a table with the following information:

• Column Name	Description
  # Doses	Displays the quantity of doses for the vaccine was ordered.
  Vaccine	Displays the Vaccine names that can be ordered from the State.
  Trade Name	Displays Tradenames that can be ordered from the State.
  Manufacturer	Displays the manufacturer's name.
  Packaging	Displays the packaging information.
  NDC	Displays the NDC number.
  Min Order Qty	Displays the minimum quantity an organization can order.
  Lot Numbers	Displays Lot Numbers that are currently in inventory for the vaccine in the row.
  Exp Date	Displays the Expiration Date for the lot number in that row for the vaccine in the row.
  Doses Given	Displays how many doses have been administered since the last order.
  Doses on Hand	Displays how many doses are currently in inventory for the vaccine in the row.
