Create Order Screen |
Purpose |
The Create Order screen is used to place a new vaccine inventory order through the system. Here you can also order Vaccine Information Statements. Note: Some vaccines may not be available for ordering. This is dependent upon whether your organization is public or private.

Create Order Section- This section displays Organization information that is specific to the Provider Organization Ordering Vaccines as described in the table below.

Displays the VFC Pin for the Provider Organization. |
Initiating Organization |
Displays the Provider Organization that started the order. |
Initiating User |
Displays the User that started the order. |
Org Phone |
Displays the phone number of the Provider Organization that started the order. |
Org Fax |
Displays the fax number of the Provider Organization that started the order. |
Order Date |
Displays the Date that the order was started. |
Delivery Contact |
Displays the Name of the Delivery Contact for the Provider Organization starting the order. |
Delivery Address |
Displays the Delivery Address specified for the Provider Organization starting the order. |
Delivery Days/Hours |
Displays the Delivery Days and Hours for the Provider Organization starting the order. |
The create order page is broken down into three sections:
Pediatric State-Supplied Vaccine - In this section the user will enter the specific Order quantities for their pediatric state-supplied vaccines.
Adults State-Supplied Vaccine - In this section, the user will enter the specific Order quantities for their adult state-supplied vaccines.
Below each individual section, the user will see a table with the following information:

Displays a text box for the user ordering vaccines to enter the quantity of doses for the vaccine in the row needed for the order that is being created. |
Vaccine |
Displays the Vaccine names that can be ordered from the Oregon Immunization Program. |
Trade Name |
Displays Trade names that can be ordered from the Oregon Immunization Program. |
Manufacturer |
Displays the manufacturer's name. |
Packaging |
Displays the packaging information. |
NDC |
Displays the National Drug Code (NDC) number. |
Min Order Qty |
Displays the minimum quantity an organization can order. |
Doses Given |
Displays how many doses have been administered since the last order. |
Doses on Hand |
Displays how many doses are currently in inventory for the vaccine in the row. |
Lot Numbers |
Displays Lot Numbers that are currently in inventory for the vaccine in the row. |
Exp Date |
Displays the Expiration Date for the lot number in that row for the vaccine in the row. |

When you click on the Submit Order button your order will be checked for errors and then submitted. You will then be returned to the manage orders screen. |

When you click on the Cancel button any entries you have made will not be saved and you will be returned to the manage orders screen. |
